*NOTE: please let us verify your insurance benefits since our therapist, acupuncturists, and doctors are in-network and out-of-network with different insurance companies. Just fax us a copy of your insurance card along with your ID number, address, date of birth, and phone number. We will verify your insurance in 1-2 business days. Fax.212-302-0989. We accept cash, Visa and Master Cards, but no personal checks.
Gift Certificate
Holistic Elements Gift Certificates are available for specific dollar amounts, specific treatments or our wonderful package programs including our facials, massage, bodywork, and acupuncture. We happily accept Visa and MasterCard over the phone. Once ordered, we can send the certificate to you by mail or even by email. You can also arrange to pick it up in person, but please call us first before you stop by. You may buy your gift certificate and make the appointment at the same time.
